Case Manager – Community Residence Program (8th Street)

Unity House of Troy is seeking a compassionate and dedicated Case Manager to join our Community Residence team located on 8th Street. If you enjoy supporting individuals on a personal level and helping them navigate day-to-day routines, this may be the perfect opportunity for you.

Key Responsibilities

+ Foster a structured, supportive, and inclusive environment for residents.

+ Build positive, professional relationships with consumers.

+ Assist with daily living skills, including cooking, budgeting, and personal care.

+ Support residents with medication management.

+ Ensure the facility remains clean, safe, and well-functioning.

+ Provide supportive counseling and offer encouragement and positive feedback.

+ Transport clients and complete program-related errands as needed.

+ Promote a culture of belonging and respect.

Qualifications

+ High School Diploma or GED required.

+ Minimum 1 year of experience working with adults living with mental illness.

+ Valid NYS Driver License and reliable transportation.

+ Strong verbal and written communication skills.

+ Basic computer proficiency.

+ Ability to work independently and collaboratively as part of a team.
