The Nurse Practitioner provides health care services to patients within scope of license, performs physical examinations and diagnostic tests, assessments and screenings, prescribes medications, and instructs patients and family members on medical conditions and treatment. Additionally, the Nurse Practitioner may have an area of specialty practice.
Minimum Requirements:
+ Current Nurse Practitioner Certification and license in the New Jersey required
+ Certification as a Nurse Practitioner by a nationally accredited nursing organization (e.g. American Academy of Nurse Practitioners – AANP, American Nurses Credentialing Center – ANCC) is preferred
+ Current and unrestricted DEA
+ Ability to work without direct supervision and practice autonomously
+ Must possess reliable transportation and the ability to travel within the designated region to meet patient needs including in-home and clinic visits as needed
+ Must be willing to become licensed in multiple states as needed
+ 2+ years working with patients in a palliative care setting required; other relevant experience will be considered
+ Prefer to have ACHPN or CHPN certificate
+ Maintain collaboration agreements, per state requirements
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or state regulation)
+ Must meet all federal, state and local requirements
+ Must be at least 18 years of age
