Job Summary:
As the Transition Care Manager, you manage all aspects of patient transition to and from the ProMedica Flower Rehab Center.
You review medical records and interact with physicians, nurses, therapists, leadership, coding and clinical documentation specialists, caregivers, and patients.

REQUIREMENTS
+ Education:Registered Nurse from an accredited school of nursing or a bachelor's prepared allied health professional.
+ Skills:Strong verbal and non-verbal communication and persuasion skills both at patient and healthcare professional levels. Ability to solve interdisciplinary problems in a team environment. Ability to input, retrieve and analyze relevant EMR data. Mid-level Microsoft Word and Excel skills.
+ Years of Experience:Minimum of 2 years clinical practice experience in a patient care setting.
+ License:Registered Nursed Licensed in the state of practice or Licensed or Registered Allied Health Professional
+ Certification: CPR
PREFERRED REQUIREMENTS
+ Education: Bachelors, Masters or Doctorate degree in a nursing or allied health profession.
+ Skills:Ability to work with a variety of internal/external customers and healthcare providers effectively and diplomatically.
+ Years of Experience:3-5 years clinical practice experience in a patient care setting.
+ Certification:Certified Rehabilitation Registered Nurse (CRRN), Allied Health Professional certification in IRF related diagnoses and/or patient management
