Job Description

As a Licensed Practical Nurse, you’ll play a key role in delivering skilled nursing care while supporting each individual’s right to independence, choice, and a vibrant life in the community.

Key Responsibilities
+ Provide nursing care, administer medications and treatments as prescribed.
+ Conduct routine assessments and monitor health conditions.
+ Accurately complete and maintain documentation, MARs, progress notes, and reports.
+ Support individuals in participating in daily routines and community activities.
+ Communicate health updates with families, team members, and medical providers.
+ Provide training and mentorship to Direct Support Professionals on health-related supports.
+ Promote dignity, respect, and self-direction for every individual.

Requirements
+ Active LPN license in the state of Minnesota.
+ Current CPR and First Aid certification.
+ Valid Minnesota driver’s license with acceptable driving record.
+ Must pass a MN DHS background study.
+ Physical ability to lift and transfer up to 60 pounds occasionally.
+ Experience with individuals with developmental disabilities preferred.
