LPN - Clinical Support Specialist
Full-Time
10am-6pm with every other weekend
Must have experience in a Long Term Care setting, and customer service.
This individual will also** **back up the Unit Manager and/or RN Supervisor** **.
LPN license required.
What You'll Do:
As a Concierger, you will be at the heart of our efforts to create a welcoming and supportive environment for everyone in our community. Your role will involve working closely with residents, families, and staff to foster open communication and enhance the overall patient experience.
Essential Responsibilities:
1. Act as customer service liaison for the facility.
2. Serve as an advocate for the residents.
3. Follow up on any concerns from residents, family, and staff.
4. Build relationships with residents through continual monitoring of overall satisfaction during their stay.
1. Manage difficult or emotional resident situations by identifying and resolving these problems in a timely manner.
2. Assists patients with overview of the unit, call bells, telephone, TV, etc.
3. Assists with menu completion as needed.
4. Demonstrates working knowledge of the inter-disciplinary functions within long term care.
5. Report all changes in the residents’ condition to the Charge Nurse & Supervisor as soon as possible.
6. Contributes to the effective communication and efficient functioning of the patient care units.
7. Contributes to the achievement of a high level of personal, quality service to promote quality of life for residents.
8. Contributes to the effective operations of the department by demonstrating dependability in job performance and attendance.
9. Collaborates with all staff to maintain the safety and security of all who reside, visit, or work in the facility.
10. Must be able to move intermittently throughout the day, proper body mechanics required--bending, stooping, turning, stretching, reaching above the shoulders, and occasional lifting are involved.
11. Must be able to cope with the mental, emotional, and physical stress of the position.
12. Must be able to observe, listen, and communicate effectively (written and oral).
13. Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, support agencies, and outside agencies.
14. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
15. Must make a neat appearance and be pleasant, helpful, and friendly.
16. Will perform other duties as required by the Administrator and Director of Nursing.
17. Welcome new residents and families, providing tours.
18. Complete nursing admission assessments (admission evaluations)
19. Ensure all admission paperwork and consent forms are completed and filed.
20. Check on newly admitted residents to ensure comfort and satisfaction.
21. Assist Supervisor with other tasks as needed.
Key Responsibilities:

+ Work collaboratively with the care team to ensure that residents' needs and preferences are met
+ Advocate for residents' rights and ensure that their voices are heard in all aspects of their care
+ Implement initiatives to improve the overall patient experience

+ Provide support and guidance to families during difficult transitions or times of need

Qualifications of an Licensed Practical Nurse (LPN) include:
+ Valid state license (LPN)
