Registered Nurse (RN) Case Manager – CorrectionalFacility

Schedule: Full-time, onsite

Pay:

+ Local: $70/hour

+ Travel: $3,164 weekly gross (including stipends)

Position Overview

We are seeking anexperienced RN Case Manager to join ourteam at a correctional facility in Aberdeen, WA. This role focuses oncoordinating and managing patient care plans, ensuring continuity of care, andsupporting the health and well-being of an incarcerated population.

Key Responsibilities

+ Develop, implement, and monitor individualized care plans for patients.

+ Coordinate medical services, referrals, and follow-up care.

+ Collaborate with interdisciplinary teams to ensure comprehensive treatment.

+ Maintain accurate documentation and comply with facility and state regulations.

+ Provide patient education and advocate for appropriate healthcare resources.

Qualifications

+ Active RN license in Washington State (or compact license).

+ Minimum 2 years of nursing experience; correctional or case management experience preferred.

+ Strong organizational and communication skills.

+ Ability to work independently in a secure environment.

The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

+ Current RN licensure in state practicing

+ At least one year of Case Management experience preferred

+ Current CPR if applicable

+ TB questionnaire, PPD or chest x-ray if applicable

+ Current Health certificate (per contract or stateregulation)

+ Must meet all federal, state and local requirements

+ Must be at least 18 years of age
