Under the direction of the Director of Pharmacy, the Ambulatory Clinical Pharmacist is responsible for developing, implementing and maintaining pharmacist-managed ambulatory care and/or specialty pharmacy services at St. Peter’s Health Medical Group as part of a team-based patient care model.
JOB DUTIES INCLUDE BUT ARE NOT LIMITED TO:
- Develops and implements drug protocols and collaborative practice agreements
- Monitors drug therapy according to established protocols and documents all patient care activities
- Collaborates with providers in medication therapy management to provide services such as drug interaction checking, side effect management, medication counseling, and optimizing medication therapy.
- Coordinates with scheduling and billing to optimize workflow, productivity, and reimbursement
- Collaborates with the outpatient retail and specialty pharmacies for benefits investigation…..
- Collaborates with designated staff to identify and pursues copayment assistance, patient assistance, or foundation assistance for patients
Collaborates with members of the care team to optimize patient outcomes
- Establishes quality measures, monitors and reports quality data, and identifies opportunities for quality improvement
- Develops educational programs for patients, providers, and staff
- Effectively markets ambulatory care and specialty pharmacy services to patients, providers, and staff
- Participates on key clinical and operational committees
- Assures compliance with clinical best practice standards and guidelines, accrediting agencies, billing guidelines, and all state and federal regulations.
- Precepts pharmacy residents and APPE students
- Responsible for oversight of the outpatient antimicrobial stewardship program in applicable clinic settings
- Performs other pharmacist duties as assigned
KNOWLEDGE/EXPERIENCE: Knowledge of drug therapy and pharmacologic treatment of disease states. Knowledge of best practice guidelines for management of chronic disease states. Knowledge and analytical skills for providing therapeutic drug monitoring, drug information, drug utilization reviews and quality assurance activities. Knowledge of pharmacist managed drug therapy protocols. Must be able to analyze results, make recommendations and follow-up on recommended actions. Must have knowledge of and ability to efficiently utilize technology in the practice of pharmacy.
EDUCATION/EXPERIENCE: Bachelors in Pharmacy or PharmD from an ACPE-accredited school of pharmacy required. Completion of an accredited ASHP Residency Program and/or experience in ambulatory pharmacy practice preferred.
LICENSE/CERTIFICATION/REGISTRY: Licensed as a registered pharmacist in Montana. Board of Pharmaceutical Specialties certification or other specialty certification required when eligible. Must become credentialed as a Clinical Pharmacist Practitioner in the State of Montana when eligible if practicing in Primary Care. Must become credentialed as a St. Peter’s Health provider and maintain credentialing per SPH medical staff by laws if practicing in Primary Care.

Demonstrated ability to work effectively with a wide variety of people with different sociological and educational backgrounds. Demonstrated interpersonal skills and writing skills necessary to communicate effectively with physicians and other care team members regarding medication use and drug information. Demonstrated high level of mental acuity and intelligence. Demonstrated high level of motivation to keep current with drug information and current best practices. Demonstrated analytical skills to determine drug appropriateness and evaluate drug interactions. Ability to collect pertinent clinical data, analyze results, make recommendations and follow-up on recommended actions. Demonstrated ability to use sound judgement in dealing with complex situations. Provides logical and effective course of action while recognizing limitations of knowledge and experience in response to issues involving pharmaceutical care or pharmacy procedures.
