In this dynamic role, you’ll provide hands-on case management and employment-focused support, guiding clients as they explore career options, secure competitive jobs, and navigate their path toward long-term success. Under the supervision of a Licensed or Qualified Mental Health Professional, you’ll play a key role in diagnostic assessments, treatment planning, and monitoring progress throughout the recovery journey. You’ll also collaborate closely with a multidisciplinary ACT (Assertive Community Treatment) team, ensuring seamless coordination of care to strengthen treatment plans, enhance safety, and help clients achieve their goals.

Responsibilities

+ Performs diagnostic assessments and collect s diagnostic information to determin e an individualized treatment plan for the client.

+ Plan s treatment modalities, coordinate s treatment plans and follow s clients through the treatment process .

+ Provides counseling, problem-solving and community support ( individual and group ).

+ Requests consultation for clients that includes but is not limited to providers, social workers , and nursing staff when necessary.

+ Evaluate the client’s treatment plan through follow-up assessment and makes adjustments based on clinical findings.

Qualifications

Education:

+ Requires bachelor’s degree with at least 30 semester hours or equivalent quarter hours in a human services field, including but not limited to sociology, social work, counseling, psychology, or human services.

License(s)/Certification(s):

+ Valid driver’s license.

+ Crisis Prevention Institute (CPI) training within 6 months of hire.

Experience:

+ Experience working with persons with severe and persistent mental illness.
