The Assessment Worker/Family Resource Coordinator responsibilities include:

+ Reviewing hospital or clinic records and interviewing families, prenatally or immediately after birth, to determine families eligible for services through the Healthy Families/Healthy Start Program.

+ Conducting comprehensive assessments within the hospital using standardized protocols and instruments and design individualized interventions for families designed to improve pregnancy and childhood health outcomes.

+ Performing other duties as assigned.

Position details:

+ **Location:** St Josephs Hospital South - Riverview Florida **AND** St Joseph's Women's Hospital - Tampa Florida

+ **Status:** Part time, 28 hours per week

+ **Schedule:** Saturday & Sunday 7:00 AM – 5:30 PM - St Joseph's Hospital South

Another weekday(flexible) - 8 hour shift - St Joseph's Women's Hospital

+ **Weekend Requirement:** Every

+ **On Call:** No

Education/Experience:

+ Bachelors Degree in Related field with 2 years’ experience working with women, children or Family Services **OR**

+ High School Diploma with 3 years’ experience with Healthy Families program
