With the creation of our new umbrella organization we now have our own identity with a unique payroll, benefits, performance management system, service recognition programs and other common practices across the system. *_Position Summary:_* The Manager of Care Management is a master’s prepared registered nurse or a master’s prepared social worker who manages the daily workflow processes, provides oversight of and directs communication among the Care Management team. The Manager coordinates team processes, administrative duties and team management behaviors. By embodying the H3W leadership behaviors, the manager supports the CM team, embracing change management, innovation and care transformation. **_Position Responsibilities:_** **Key Areas of Responsibility** *Manager Responsibilities:* · Support the day-to-day operations of Care Management teams, in partnership with ambulatory care management leadership team.** · Educate, implement, and monitor adherence to standard work processes · Operationalize strategic initiatives to support team performance in outcome measures, KPIs, and workflow management · Represent ICP at meetings, community, provider and payor collaboratives · Support talent management · Manage within required budget requirements · Foster change management efforts and innovations which advance CM practice. · Monitor day-to-day activities of staff in accordance with applicable policies and procedures, ensuring protocols are upheld. · Teach, coach, mentor, facilitate and collaborate as ICP representative · All Other duties as assigned *_Working Relationships:_* *This Job Reports To:* Director of Care Management **# Workers Responsible For:** Direct Reports: 20-30 Indirect Reports: 10-20 *Nature of Supervision:* Daily support and supervision of Care Management team, including Community Care Managers and Resource Coordinators to manage and maintain workflow, uphold policy and procedure and drive patient centered care.**** *Internal:*Care management teams** *External:*External customers of Integrated Care Partners, system based stakeholders requiring support of ICP Clinical team**** *_Requirements and Specifications:_* **Education** · Minimum: Bachelor’s of Nursing or Master's of Social Worker. · Preferred: Master’s degree, or LCSW. **Experience** · Minimum: o 5 years clinical experience o 5 years case management experience, community based preferred, hospital environment considered o Previous supervisory experience required · Preferred: o 10 years Clinical Experience o 10 years Case Management experience § Community Health experience highly desired § Case Management experience highly desired § Payer Based Care Management experience highly desired **Licensure, Certification, Registration** · Minimum: o Valid Connecticut Nursing License o Valid Connecticut Social Work License · Preferred: CCMC/ACMA/CMSA preferred **Language Skills** · Minimum: Mastery of English language**** · Preferred: Ability to communicate in multiple languages highly desirable**** **Knowledge, Skills and Ability Requirements:** · Engaging personality and comfort with interpersonal interactions, public speaking, public forums · Willingness to develop new skills and have ability to receive feedback · Strong computer skills, including solid working knowledge of Microsoft Office software · Positive customer service focused approach · Proven ability to work effectively independently and in a team environment · Must be comfortable and flexible with ambiguity and change processes · Excellent written and verbal communication skills · Ability to communicate effectively · Strong organization skills and attention to detail · Demonstrated ability as fostering and facilitating clinical based research competence · Demonstrated ability to foster collaborative relationships · Must successfully pass Chronic Disease Certification Course and Exam as part of orientation process · Must successfully achieve Mental Health First Aid certification as part of orientation process *_We take great care of careers._**__* With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
