Join Our Team as a Social Worker (MSW) – Blank Craniofacial Clinic Make a meaningful impact by supporting patients and families through complex care needs. As a Licensed Master’s Social Worker, you’ll collaborate with a multidisciplinary team to provide biopsychosocial assessments, advocacy, crisis intervention, discharge planning, and care coordination. Your expertise will help address psychosocial factors that influence health, facilitate education, and guide families through transitions of care—all while promoting self-determination and adjustment to illness.

Responsibilities
Patient Care Coordination:
Identifies patient needs by assessing psychosocial, environmental, financial and cultural strengths and barriers and collaborates with UPH, community services, and agencies to support patient safety and continuity of care, as applicable. As a member of the interdisciplinary team, contributes to the comprehensive episodic and/or longitudinal plan of care based on patient-centric goals and coping strategies. In select environments, completes comprehensive mental health assessments utilizing screening tools in alignment with the DSM-5 criteria. Maintains comprehensive knowledge of community resources and acts as a liaison to refer patients/families to on-going community based services, provides education on application for health insurance and disability services, public assistance and other resources to meet patient identified needs. Documents assessments, interventions, and referrals in the electronic health record according to documentation standards. If working in a hospital setting, participates in interdisciplinary care rounds as indicated by area of specialty
Education and Advocacy:
Serves as a patient/family advocate in support of patient confidentiality, informed consent, patient autonomy, and self-determination. In collaboration with the multidisciplinary team, assesses patient safety in regards to possible abuse, neglect or other risks to safety. In accordance with mandatory reporting guidelines, files abuse/neglect reports to the appropriate state agency and/or guides others in the process as mandated. Supports culturally competent services and assists with arranging interpreter services as needed. Provides education to the patient/family regarding available services and supports and assists the patient to access those they are eligible for. Provides information and education to providers and other team members in understanding the psychosocial implications of illness and disease progression for the patient/family. Educates professionals within the healthcare setting about issues related to psychosocial functioning and case management activities. Participates in mentoring new team members and/or supervising social work students as applicable.
Quality:
Supports initiatives to reduce disparities to advance healthcare equity strategy. Participates in evaluation of standardization in documentation to ensure consistency in care delivery. Participates in quality improvement projects as relevant to the position.
Qualifications
Education: Masters degree Required in Social Work from an accredited school of social work. Work Experience: Preferred 2 years relevant social work experience in a healthcare setting or similar. Licenses and Certifications: LMSWIA, LMSWIL, APSWWI. IA Lic Master Social Worker, IL Lic Master Social Worker, WI Advanced Practice Social Worker REQUIRED. LISWIA, LISWIL Licensed Independent Social Worker (LISW) PREFERRED. Iowa Department of Public Health. IDPH. Illinois Department of Finanical and Professional Regulation. IDFPR within 180 Days Required - MRCHILDIA IA Child Abuse Mandatory Reporter Training in state(s) where providing care. Upon Hire. Required - MRCHILD-ILDCFS IL Child Abuse Mandatory Reporter Training in state(s) where providing care. Upon Hire Required - MRDEPADULTIA IA Dependent Abuse Mandatory Reporter Training in state(s) where providing care. Upon Hire Required - MRDEPADULT-ILDOA IL Dependent Adult Abuse Mandatory Reporter Training in state(s) where providing care. Upon Hire Required - MOAB, Advanced De-Escalation Completion of Advanced De-Escalation course. Management of Aggressive Behavior (MOAB) may be required depending upon the specialty area. within 90 Days BCLS Cert Basic Life Support American Heart Association, AHA Upon Hire. Preferred.
