What You'll Do

As our RN Clinical Manager, you'll play a key role in ensuring high-quality, compassionate home health care. Your leadership helps keep our nurses supported, our clients safe, and our mission thriving.

+ Collaborate on creating, reviewing, and updating patient plans of care

+ Provide nursing care according to physician orders and client needs

+ Document and report changes in patient condition

+ Educate and guide clients, families, and staff

+ Participate in QA programs, education sessions, and team meetings

+ Assist with assigning staff, reviewing referrals, and case coordination

+ Begin discharge planning from day one and complete discharge summaries

+ Conduct HHA supervisory visits and PRN visits as needed

+ Work rotating weekends and take on-call shifts when scheduled

+ Travel locally to patients’ homes and outreach locations

+ Maintain compliance with all Phoenix policies and regulatory standards

+ Support the DON in daily communication regarding patient care

+ Perform other leadership duties as assigned

What You Bring

+ Active Missouri or Compact RN License

+ 1+ year of RN experience (Home Health preferred)

+ Valid driver’s license & current auto insurance

+ Current CPR certification

+ Ability to lift 50 lbs

+ Strong communication, time-management, and leadership skills
