Position Overview:
Provides evaluation, treatment and follow-up to patients within a clinical nursing program. Serves as a primary point of contact for all things related to the care and education of the patients and ensures efficient and effective processes that support safe patient care throughout continuum of care. Participates in the development and implementation of clinical programs and procedures and participates in performance improvement programs. Coordinates and supervises nursing standards of practice for patients and participates in data reporting. May also be responsible for performing specific procedures and/or orient other staff to the department.

JOB ACCOUNTABILITIES:
.PATIENT CARE.
• Conducts patient assessment encompassing physiological, spiritual and emotional areas.
• Monitors patient care interventions, assesses therapies, evaluates outcomes, and intervenes as warranted.
• Collaborates and communicates with physicians and other members of the healthcare team to interpret, adjust, and coordinate patient care plan to ensure a plan of care that is patient centric with continuity of care.
• Partners with peers, other healthcare providers, and management to effectively streamline patient workflow, improve patient outcomes, and provide the highest care quality.
• Communicates patients’ plan of care to interdisciplinary care teams and between departments.
• Provides inpatient and outpatient education in the form of individual consults, conducts classes and support groups for patients and the community at large.
• Delivers safe and effective patient care in compliance with established policies and procedures, as well as local, state and federal regulations to provide the highest level of patient care.
• Upholds the confidentiality of all patient protected health information (PHI) and protects all PHI from accidental, intentional or inappropriate disclosure.
• Accurately documents and updates all treatment information in the patient’s electronic health record (EHR).
• Remains current in advancements, trends, and best practices related to acute care and area of specialty and in professional nursing practice.
CLINICAL PROGRAM COORDINATION.
• Provides clinical oversight and directs clinical nursing program services and systems to facilitate an organized, multi-disciplinary team approach to patient care.
• Coordinates the entry and integration of patients into and through the clinical program in assigned area.
• Responsible for the implementation, integration and clinical support for quality assurance, performance, performance improvement and risk management activities in assigned area.
• Acts as a resource for members of the interdisciplinary team on care and protocols to promote positive outcomes for patients.
• Coordinates and supervises nursing standards of practice for patients and manages data reporting.
EDUCATION.
• Work closely with patients, families, physicians, and members of the outpatient and inpatient nursing staffs, and other health care team members in formulating instructional patient care and teaching plans.
• Uses knowledge of specific clinical needs when developing educational materials.
• Provides information regarding diagnosis, procedure, and post-procedure issues to patients, families, donors, and caregivers.
• In conjunction with nursing staff, medical staff, and management, establishes a clinical program education program for orientation and continuing education.
• Uses established educational methods.
DATA AND ANALYSIS.
Assists in designing plans for clinical program data gathering and analysis.
Actively participates or may lead in analyzing data and developing recommendations and action plans.
EDUCATION:
Other: Graduate of an accredited school of nursing.
CERTIFICATION & LICENSURE:
RN-Registered Nurse for the state where care is provided
DEPARTMENT REQUIRED CERTIFICATION & LICENSURE
Department, SMCS Lactation Services: IBCLC-International Board Certified Lactation Consultant
TYPICAL EXPERIENCE:
2 years recent relevant experience.
SKILLS AND KNOWLEDGE:
Demonstrated knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology.
Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record (EPIC preferred).
Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
Ensure the privacy of each patient’s protected health information (PHI).
Build collaborate relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

Pay Range is $86.68 to $114.41 / hour
_The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package._ __
