Natchaug Hospital is Eastern Connecticut’s leading provider for children, adolescents and adults with mental illness and substance abuse addictions. We offer a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools and chemical dependency services. Through our multi-site network of care, we constantly strive to meet its mission in helping people find their way while educating and empowering individuals to participate in their own care and recovery. *_Program Summary_* Natchaug Hospital's Journey House program is a 13-bed specialized residential treatment facility on the Mansfield campus for adolescent girls ages 12 to 18 who are involved in the court-system. Referred girls have identified needs for behavioral health services and may have a history of one or more of the following circumstances - significant trauma, mental illness, victim of Domestic Minor Sex Trafficking, a history of substance abuse, and assaultive and/or self-injurious behaviors. *_Job Summary_* Community and Vocational Coordinator works directly with residents and their families in the community and within the program while in placement at Journey House. They will be responsible for assessing and building vocational skills of the residents and partnering with the families to follow up on therapeutic goals at discharge. They will be accountable to continue to work on discharge goals and vocational skill development through 90 days post discharge. _Key responsibilities:_ * Works with the clinical team (Clinicians, Mental Health Workers, Nurses, etc.), in the development and implementation of treatment plans, both during placement at the Journey House and after discharge. Conducts life skill assessments and develops a plan to improve those skills. (ARISE model). * Transports residents and families during placement at Journey House and during the aftercare follow up period. * Facilitate Dialectical Behavior Therapy (DBT) and other psycho-educational groups. * High school diploma required, Bachelor’s preferred * 4 years’ experience in Behavioral Health environment * Ability to obtain a Public Service License “A” endorsement within 90 days of hire, which requires you to pass pre-hire DOT physical and drug screen. * Must have a minimum of 3 consecutive years of a valid driver's license with good driving record. * Ability to obtain Medical Administration Certification and Crisis Prevention Certification within 90 days of hire * Bi-lingual in English/Spanish a plus * Ability to perform physical interventions * Ability to work flexible hours including evening and weekend coverage.
