Will travel with the Neighborhood Health team, HHCs mobile healthcare program, which provides mobile healthcare clinics embedded within community locations to ensure all community members have access to equitable healthcare. Neighborhood Health team members meet at our satellite office locations and travel together in HHC fleet vehicles for mobile clinics. *_Job Description_* The Community Care Manager is an integral member of the Neighborhood Health team who provides in-person care management to patients of Neighborhood Health. The Care Manager contributes to meeting the health care, safety and comfort needs of patients and are performed in accordance with all existing regulatory, state and company standards, policies and procedures. Provides excellent customer service and support; exhibits strong active listening skills, as well as excellent written and verbal communication.. Key areas of responsibility • Responsible for patient navigation, addressing barriers impacting patient health and well-being. Connects patients to resources to address positive Social Determinants of Health (SDOH) screenings. Including, but not limited to, navigating the 211 system, accessing local food pantries and soup kitchens, obtaining transportation to medical appointments, and more. • Responsible for in-person care coordination through connections to primary, specialty and behavioral health care, in collaboration with the onsite provider. Also responsible for ongoing care coordination as needed based on the client’s individual needs. • Leads inpatient referral to street medicine program, managing all referrals from inpatient care management and social work departments by updating tracker and scheduling inpatient visits. Offers collaboration with the inpatient CM/SW and street medicine teams with consideration to unique medical and social needs. Engages with community resources to make post discharge connections as needed. Educates new team members on initiative and standard process. • Responsible for managing and updating repository of community resources, including organization name, location, scope of services and contact information. This repository serves as a team reference to ensure awareness of local offerings and connections for patients. • Provides education for patients, families, community, and other professionals regarding disease prevention, impact of illness and advocacy for benefits and health maintenance • Initiates appropriate action in urgent situations by adjusting priorities to address patient and/or colleague immediate issues. • Responsible for driving fleet vehicles to and from mobile events. • Responsible for restocking mobile equipment inventory • Transports of mobile equipment bins alongside team members. Load mobile equipment onto vehicles prior to office departure, offload upon arrival at location, load back onto vehicles at close of event and offload upon return to office. • Coverage required at multiple satellite locations • Perform other related duties as required Master of Social Work (MSW) or other Master’s-level in related field OR 3-5 years of experience in a related field, in lieu of education. LMSW/LCSW/CHW preferred. Community health work experience preferred. Valid Driver’s license within 3 Months (90 days) of hire date or job transfer date Ability to push/pull/lift 50 lbs.
