As a Geriatrics Patient Navigator, you will collaborate with an interdisciplinary care team to optimize patient access to the Center for Geriatric Medicine. In this role, you will provide targeted, high-touch navigation services for high-risk patient populations, proactively identifying and closing care gaps. You will triage urgent patient and caregiver concerns and coordinate timely solutions with geriatrics staff and community partners to ensure seamless, patient-centered care.
A caregiver in this role works days from 8:00 a.m. - 5:00 p.m.
+ A caregiver who excels in this role will:
+ Serve as a single point of contact for the scheduling needs of new and established patients of the Center for Geriatric Medicine.
+ Identify and resolve access challenges for high-risk patients including communicating any special instructions or directions for visits and scheduling virtual visits for patients who are unable to be seen in the office.
+ Connect patients with other Cleveland Clinic and community resources and facilitate completion of referrals using effective communication skills.
+ Conduct targeted outreach to a panel of high-risk patients to ensure timely and efficient delivery of care related to the geriatrics 4Ms (what matters, mentation, mobility and medication).
+ Assist in managing transitions of care across care settings, ensuring optimal communication and planning.
+ Advocate for patients and caregivers and facilitates resolutions for patient/caregiver questions and concerns.
+ Maintain longitudinal relationships with patients and caregivers.
+ Support quality and process improvement initiatives.
Minimum qualifications for the ideal future caregiver include:
+ High School Diploma or GED **and** three years of recent experience working with patients in a clinic setting or other equivalent position, including interacting with patients, physicians, and other departments in a collaborative manner
+ **OR** Associate's or Bachelor's Degree **and** two years of experience
+ Knowledge of data collection, analysis, and proficient in Microsoft Office
+ Experience in providing exceptional customer service and conflict resolution
+ Working knowledge of HIPAA regulations
Preferred qualifications for the ideal future caregiver include:
+ Associate's or Bachelor's Degree
+ Knowledge of medical terminology
Physical Requirements:
+ Must be able to sit for prolonged periods of time.
+ Possess good visual acuity through normal or corrected vision.
+ Possess dexterity to operate a PC and other office equipment.
+ Light driving to outside locations within the region.
Personal Protective Equipment:
+ Follow standard precautions using personal protective equipment as required.
Pay Range
Minimum hourly: $17.25
Maximum hourly: $26.31
