**Location:** Valley View Family Medicine Clinic - Council Bluffs, IA

**Schedule:** Monday-Friday 8a-5p

CHI Health is seeking a passionate and skilled Clinic Wellness Registered Nurse (RN) to provide exceptional, whole-person care in our dynamic family medicine clinic. This role is perfect for a professional who thrives in a collaborative environment and values building lasting relationships with patients and their families. + Work as part of a team to provide Annual Wellness visits to current Medicare patients past their 1st year of eligibility.

+ Obtain and document health and family history, information on chief complaint, vital signs, health screenings, and yearly goals.

+ Collaborate with the patient and provider to create custom 5-10 year preventative screening plans for Medicare patients.

+ Provide education to patients and families regarding health status, the importance of health maintenance/preventive care, and management of acute and chronic conditions.

+ Facilitate conversations on advance care planning.

+ Other duties as assigned.

Job Requirements

Minimum Qualifications:

+ Graduate of accredited school of nursing.

+ Bachelor of Science in Nursing.

+ Current/valid Registered Nurse License from the state licensing board required.

+ Basic Life Support (BLS) from the American Heart Association required upon hire.

+ 1 year related experience in a clinic, specialty clinic or acute hospital setting preferred.

Pay Range

$27.97 - $41.60 /hour
