What You’ll Do:

+ Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.

+ Provide psychosocial assessments for clients.

+ Develop individualized treatment plans for youth, adults, and families.

+ Implement patient care plan activities based on treatment goals towards independent living for the patient/family.

+ Serve as resource for staff education & training, participate in case conferences as needed.

What You’ll Bring:

+ Master’s degree from a school of social work accredited by the council on social work education.

+ Maine Board of Social Work Licensure: LCPC, LCPC-C, LCSW or LMSW-CC

+ Current CPR card required.

+ One or more years of experience as a master’s level social worker in a health care setting.

+ A valid Drivers License and reliable car with insurance.
