Position Description
The Metro Nashville Public Health Department (MPHD) is seeking an experienced Public Health Nurse with the passion and experience required to assist persons diagnosed with Tuberculosis (TB) or suspected of having TB. This position will serve as a case manager for patients diagnosed with TB infection, TB disease or suspected of having TB.
The TB Elimination Program works with community agencies, providers, and hospitals to identify persons who have TB infection, active TB disease and suspected TB. Diagnosis, treatment, and follow-up services are provided to all persons in Davidson County who are diagnosed with active TB disease. Contact investigations are conducted in partnership with patients who have active TB disease to identify, evaluate, and treat individuals who have been exposed to active TB. Directly observed therapy is provided to all persons in Davidson County with TB disease, children, and some high-risk contacts.

Typical Duties
+ Serves as a nurse case manager for patients diagnosed with TB Infection, TB Disease or suspected of having TB.
+ Assists with the care of patients in hospitals, nursing homes, and long-term care facilities.
+ Prepares reports and presentations for audits.
+ Tracks and monitors the status of interjurisdictional (IJ) forms.
+ Recommends and participates in implementing service and organization modifications to ensure achievement of program goals and objectives.
+ Coordinates care for TB patients with MPHD’s team of medical providers.
+ Tracks and follows up on referrals to specialists.
+ Prepares and discusses patient cases with the MPHD TB staff.
+ Communicates with the staff of the Tennessee TB Elimination Program (TTBEP) and personnel representing other regions as necessary for patient care.
+ Documents physical charts, PTBMIS and other electronic databases.
+ Learns and follows TB treatment guidelines and protocols.
+ Assists in the TB Elimination clinic with other patients as directed by the supervisor.
+ Participates in training, conferences, outreach events and screenings.
Minimum Qualifications
Diploma or Associate’s Degree in nursing and one (1) year of nursing experience preferably in the field of public health. Candidates with Diploma or Associate’s Degree in nursing must establish a documented ability to competently perform all functions of this classification to be considered. Bachelor’s degree from a state-approved school of nursing preferred. All public health nurses are required to complete a department approved Certificate in Public Health as a condition of continued employment. Possess required competencies upon hiring.
Licensure:
+ Must be Licensed as a Registered Nurse in the State of Tennessee. Applicants holding a Registered Nurse license from a state that participates in the “Enhanced Nurse Licensure Compact” may be hired, but must obtain their Tennessee license within thirty (30) days of hire.
All public health nurses are required to complete an MPHD-approved Certificate in Public Health as a condition of continued employment.

Preferred Experience, Knowledge, Skills, and Abilities
+ Knowledge of TB treatment guidelines and protocols.
+ Comfortable with making case presentations before a public health team.
+ Bilingual (Spanish).
