    Minimum Qualifications:

    Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science. Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counselingwith2 years of MH case management experience

    Job Description:

    This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.

    Job Duties:

    + Collaborate with program clinicians to coordinate ongoing care for TCHATT patients.

    + Act as liaison between patients and clinicians.

    + Disposition TCHATT cases upon discharge from services.

    + Develop and coordinate care plans for patients.

    + Assist with development and maintenance of the mental health resource database.

    + Demonstrate competency and skill in communicating with patients, families, and school personnel.

    + Participate in team meetings and provide status updates to team members and leadership.

    + Participate in conferences, workshops, and trainings related to your role.

    + Assist with maintaining project data and required reports.

    + Other duties as assigned.

    Preferred Qualifications:

    + 1 year experience working with families.

    + Must have proficient computer skills.

    Salary Range:

    Commensurate with experience.
