Physical Therapist Assistant (PTA)

Part-Time (24 hrs/week)

Home-Based Care | South Lyon • Plymouth • Walled Lake

About the Opportunity

We’re looking for a part-time Physical Therapist Assistant to join our home health team and support patients across South Lyon, Plymouth, and Walled Lake. You’ll partner closely with a supervising Physical Therapist to help patients build strength, increase mobility, and improve their overall quality of life.

What You’ll Be Doing

+ Provide hands-on therapy interventions according to the established Plan of Care.

+ Support patients in achieving greater independence through mobility training, strengthening exercises, and safe use of assistive equipment.

+ Monitor patient progress and alert the supervising PT to changes in condition or care needs.

+ Deliver care in alignment with state practice regulations and Elara Caring standards.

+ Teach patients and caregivers exercises, safety strategies, and principles for success at home.

+ Engage in physically active work—lifting, transferring, and assisting patients as appropriate.

What You Bring

+ Graduate of an accredited PTA program recognized by the American Physical Therapy Association.

+ Active, unrestricted PTA license for the state of Michigan.

+ At least six months of PTA experience (home health experience a plus).

+ Reliable transportation with valid driver’s license and current auto insurance.

+ Ability to travel within the service area (up to 50%).

+ Physical capacity to lift 50–100 lbs and perform frequent movement throughout the day.
