Speech-Language Pathologist (SLP) – Home Health

Serving Oakland County & Wayne County|

Your Responsibilities

In this role, you will:

+ Complete speech, language, cognitive-communication, and swallowing assessments

+ Develop individualized care plans tailored to each patient’s goals

+ Provide therapy services based on physician orders and best-practice guidelines

+ Coach patients and caregivers on exercises, techniques, and home programs

+ Document your visits clearly and promptly

+ Partner with nurses, therapists, and physicians to ensure seamless care

+ Promote safety and help reduce avoidable hospitalizations

What You Bring to the Team

+ Master’s degree in Speech-Language Pathology

+ Active SLP license in Michigan (or Educational Limited License with required supervision)

+ At least one year of SLP experience; home health or hospice is a plus

+ Strong communication and organization skills

+ A patient-first, compassionate approach to care

+ Reliable transportation and valid driver’s license
