Summary The incumbent is the Coordinator for the Non-Institutional Care (NIC) programs. The incumbent is responsible for general oversight of the Homemaker/Home Health, ADHC and Respite programs. The oversight includes monitoring agency compliance with VA standards of documentation and care provision as well as monitoring of VA Non-Institutional Care staff to ensure timely reviews of request for services and renewals. The incumbent serves as the subject matter expert for the designated NIC Programs. Responsibilities Total Rewards of a Allied Health Professional Duties of the position include but not limited to: The Social Worker (Program Coordinator) has responsibility for the Non-Institutional Care (NIC) programs. The Social Worker/Program Coordinator is responsible for general oversight of the Homemaker/Home Health, ADHC and Respite programs. The Social Worker/Program Coordinator provides administrative, technical, and educational consultation necessary for accomplishing the work of the program, including education to VA staff, and outreach efforts to community partners. The Social Worker/Program Coordinator is responsible for the development, evaluation, and communication of standards, policies, and procedures to the interdisciplinary health care teams who serve as primary referral sources for the program. The Social Worker/Program Coordinator attends and/or participates in a variety of monthly and quarterly conference calls disseminating needed information to the VA staff and community agency staff. The Social Worker/Program Coordinator will prepare any necessary reports such as JPSR (patient safety reports), Issue Briefs, and assist with Congressional response regarding their assigned programs. The Social Worker/Program Coordinator develops and implements systems to track a current list of active patients, service activities, dates of authorizations, utilization of services, and patients satisfaction and develops plans of correction for the programs. The Social Worker/Program Coordinator is responsible for GEC program reports on assigned programs to local management, VISN and VACO. The Social Worker/Program Coordinator will seek guidance from VISN and VACO program offices as needed and directed by supervisor. The Social Worker/Program Coordinator evaluates programs progress, outcomes, and utilizes data to continuously modify and improve the program and processes. The Social Worker/Program Coordinator will serve on related committees, or workgroups as assigned to represent assigned programs and facilitate collaboration with other stakeholders. The Social Worker/Program Coordinator will perform clinical functions as required by program policy to include assessment, providing direct clinical services to program enrollees, and communication with family, caregivers, and contracted agencies staff to address and resolve patients concerns. The Social Worker/Program Coordinator will ensure that enrollees in these programs meet administrative and clinical eligibility requirements and are receiving the appropriate level of services to meet their needs in accordance with program mandates. The Social Worker/Program Coordinator will engage in continuous quality improvement of assigned programs. The Social Worker/Program Coordinator will provide support and assistance to other NIC Social Work staff to address and assist in program workload as needed. The Social Worker/Program Coordinator is responsible to the GEC Supervisory Social Work Supervisor, but practices independently, exercising initiative and independent judgment in day-to-day activities. The Social Worker/Program Coordinator uses sound judgement and critical thinking in the interpretation of trends, patterns, the application of guidelines, and the recommendation of program enhancements. This requires a high and varied level of professional expertise, as well as strong teamwork and communication skills. In unusual situations and clinical problems, The Social Worker/Program Coordinator initiates consultation with the supervisor. Work Schedule: Monday through Friday, 8:00am - 4:30pm, Tour of duty subject to change at the discretion of Supervisor. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determination Social Worker (Program Coordinator), GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. (4) Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Preferred Experience: Experience with inpatient/outpatient medical setting. PHYSICAL REQUIREMENTS: Incumbent must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the social worker or others. Position requires walking throughout the clinic and/or Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans, caregivers, and interdisciplinary teams. Position requires minimal travel into the community to meet with community agencies and partners who assist and support the Caregiver Support program, its Veterans, and caregivers, as well as home visits with caregivers and Veterans. Incumbent must be able to operate a vehicle safely and have a valid driver's license and auto insurance. All vehicular operation related courses in TMS must be kept current. Incumbent must also have transportation to/from locations of Caregiver Support Program fleet vehicles. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer monitor/keyboard; extensive reading; and repetitive motions of the wrists, hands, Majority of work is completed in an office setting. The work of this position is primarily sedentary with occasional walking of short distances. No heavy lifting required. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situation, able to shift priorities based on patient needs. Must complete a physical as a part of the pre-employment process.
