The Clinical Manager – Home Healthat the VNH Provides leadership, consultation and collaboration in a dynamic, multidisciplinary environment and collaborates with administrators and other members of the health care team to design evaluate and improve patient care systems and processes. May include direct patient care when needed.
The Clinical Manager – Home Health:
* Manages the hiring process for hospice clinicians, including staff selection and onboarding to department.
* Evaluates and supervises performance of Home Health clinicians, to include shadow visits, performance reviews and coaching and performance management processes as outlined in organizational policies and procedures.
* Collaborates with Directors of Home Health and Hospice in planning adequate staffing and adjustments on a daily basis, based on patient needs and staff skillset.
* Supports interdisciplinary and shared-decision making by fostering an environment with open communication and assure decision making at the point of service.
* Facilitates change by assessing readiness, involving staff in change processes, communicating changes and evaluating outcomes.
* Participates in the operational, administrative, and personnel responsibilities in providing services in a 24/7 work environment.
* Directs day-to-day operations in accordance with established standards of practice, policy, and regulatory requirements.
* Plans adequate staffing and adjustments on a daily basis, based on patient needs and staff skill mix.
* Contributes to the development of annual budget; monitor patient revenues, clinical, and operational expenses and staff productivity. Reports variance to Director and takes appropriate actions to meet or exceed budget targets.
* Monitors key performance indicators and creates an action plan to improve results.
* Facilitates and monitor activities related to the delivery of patient care, assuring evidence-based practice and minimum clinical practice standards are achieved in the clinical area.
* Shares rotation of Administrative On-Call (AOC) responsibilities
Minimum Qualifications:
* Bachelor’s degree in professional clinical discipline (RN, PT, OT, SLP, MSW) from an accredited college or university, Master’s degree desired. Certification in specialty preferred.
* Two (2) years’ experience in a direct supervisory role.
* Experience in health care setting with working knowledge of medical terminology, electronic medical record and billing systems/software, home care experienced desirable
* Must have excellent interpersonal, communication and analytical skills
* Software skills: Electronic medical record, Microsoft Excel, Word, Outlook, PowerPoint, etc.
* Professional licensure required for VT and NH
* OASIS certification required within one year of employment
Salary Range (depending on experience):
* Minimum Pay:$47.50
* Maximum Pay:$66.75
