    Job Summary and Responsibilities

     

    As a Primary Care Liaison RN, you will be an integral part of our primary care team, operating to deliver comprehensive, team-based patient care. You will foster a patient-centered Medical Home environment, collaborating closely with physicians and staff while also providing leadership to LVNs and MAs.

     

    Every day you will utilize protocol-driven standing orders for a wide range of tasks, including evaluating lab data, providing patient education, performing phone triage, and assisting with medication modifications. You will actively coordinate care by facilitating diagnostic testing, ensuring timely follow-ups, and managing disease education, alongside contributing to process improvement and resource coordination.

     

    To be successful in this role, you will expertly apply your RN scope of practice autonomously and collaboratively within the primary care setting. Your exceptional communication, organizational skills, and commitment to patient advocacy will be vital for effective care coordination, team leadership, and optimal patient outcomes.

     

    + Assesses patient and family needs, unmet needs, strengths and assets.

    + Applies knowledge of the philosophy/ principles of comprehensive, community based, family-centered, developmentally appropriate, culturally sensitive care coordination services as evidenced through assessment and development of patient centered care plans.

    + Facilitates patient and family access, as appropriate, to medical home providers, staff and resources.

    + Assists with or promotes the identification of patients in the practice with special health care needs or patients at high risk; Adds to registry and uses to plan and monitor care.

    + Initiates patient and family contacts; creates ongoing processes for patients and families to determine and request the level of care coordination support they desire at any time especially during exacerbation of chronic disease.

    + Develops care plan with patient, family, and team (emergency plan, medical summary and action plan as appropriate).

    Job Requirements

    + Graduate from an accredited school of nursing and Three (3) years’ experience as an RN

    + A minimum of one (1) year in the last 5 years as an RN in an ambulatory community involving care management and/or chronic disease management (including outpatient community health clinics, private physician offices, home health and hospices, public health settings, free-standing urgent care clinics) or primary care setting

    + Current and valid Registered Nurse: CA license

    + Basic Life Support - CPR certification from American Heart Association

    Preferred

    + Bachelors Of Science Nursing or other advanced clinical degree

    + Five years of clinical patient care experience as an RN in a patient centeredprimary care setting

    + Experience in care coordination and chronic disease management

    + Accredited Case Manager

    + Case Management Nurse

    + Certified Case Manager

    Pay Range

    $56.80 - $67.52 /hour
