Job Description
The Opportunity:
Saint Luke’s is seeking a compassionate Palliative Care Nurse to serve at Saint Luke’s South Hospital. This part-time position will give you the opportunity to help patients and families at a very vulnerable time, supporting them through their goals of care and symptom management needs. You will be a part of a palliative care team that will work together to provide support to medical professionals in managing hospital inpatients or those benefiting from comfort care measures.
The Work:
Your skills as a Palliative Care Nurse will be challenged in a meaningful way as you will be responsible for:
+ Receiving pending referrals and palliative consults
+ Collaborating with a variety of team members including hospital staff, providers, post-acute care team members and the inpatient Palliative Care team members
+ Triaging patient needs
+ Providing goals of care conversations and resources
+ Educating staff, physicians, patients and families on palliative and/or end of life care
+ Facilitating transitions to next level of care, working especially with St. Luke’s Hospice and Hospice House staff
+ Collecting, reporting, and monitoring key performance indicators of Palliative Care services
+ Looked to as the subject matter expert in palliative and end of life care in the inpatient setting
Missouri and Kansas RN License required.
Job Requirements
Applicable Experience:
3-5 years
Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various
Bachelor's Degree
