    Starting salary range between $55,640 - $67,695 based on experience.

    Responsibilities

    Assists residents and families in adjusting to physical, personal psychological and environmental difficulties which may interfere with quality of life. Provides guidance and advice to medical and administrative personnel regarding the impact of psychosocial factors on the treatment and progress of residents. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to improved health.

    Essential Duties

    1. Collaborate with other professionals to evaluate patients' medical or physical condition

     

    and to assess client needs.

     

    2. Refer patient, client, or family to community resources to assist in recovery from mental

     

    or physical illness and to provide access to services such as financial assistance, legal

     

    aid, housing, job placement or education.

     

    3. Counsel residents and resident families to help them overcome dependencies, recover

     

    from illness, and adjust to life.

     

    4. Utilize consultation data and social work experience to plan and coordinate client or

     

    patient care and rehabilitation, following through to ensure service efficacy.

     

    5. Advocate for residents to resolve crises.

    6. Collaborates with residents, families and staff regarding the development of an

     

    appropriate admission and discharge plan of care.

     

    7. Organize support groups or counsel family members to assist them in understanding,

     

    dealing with, and supporting the client or patient.

     

    8. Modify treatment plans to comply with changes in clients' status.

    9. Monitor, evaluate, and record client progress according to measurable goals described

     

    in treatment and care plan.

     

    10. Identify environmental impediments to client or patient progress through interviews and

     

    review of patient records.

     

    11. Investigate abuse or neglect cases and take authorized protective action when

     

    necessary.

     

    12. Conduct social research to advance knowledge in the social work field.

    13. Participate in care conferences, consulting with the team caring for the resident, to

     

    evaluate the resident's needs and plan for continuing services.

    15. Attend and participate in all appropriate meetings and training opportunities in order to

     

    keep informed on current information and skill development.

     

    16. Follow established policies and procedures including but not limited to:

    Qualifications

    Education :Bachelor's Degree in Social Work or related field

     

    Experience :None, but experience in a geriatric environment is beneficial and preferred

    Computer Skills:

    Must be able to operate a computer Proficient in Microsoft Office Software

    Certificates & Licenses:

    MI Social Worker License

    Ability to work flexible hours including occasional evenings and weekends
