Occupational Therapist
Job Description:
VIA is seeking an Occupational Therapist to work on a per diem basis. Reporting to the Vice President of Vision Rehabilitation, the Occupational Therapist is primarily responsible for providing quality, vision and person-centered OT services to all ages of individuals experiencing functional decline due to vision loss.
This per diem position is paid on a fee-for-service basis at a rate of $20.00 per 15-minute direct-care service unit.
Essential Functions:
+ Conduct thorough assessments of patients' visual, cognitive and functional abilities and develop personalized treatment plans
+ Provide one-on-one therapy sessions to help patients improve their daily functioning
+ Collaborate with interdisciplinary team to coordinate patient care and ensure positive outcomes
+ Educate patients and their families on adaptive equipment, assistive technology and other tools to maximize residual sight
+ Keep detailed records of patient progress and modify treatment plans as needed
+ Document progress and outcomes with appropriate record management, including reporting and data entry to both internal and external databases in accordance with established guidelines, including patient discharge in timely manner
+ Transports self to patient homes or facilities within VIA’s service area for the purpose of preserving patients’ independence and obtaining goals of staying in place
+ Order adaptive equipment in accordance with internal guidelines and seeking appropriate authorization from Manager when applicable, dispense and train

Qualifications:
+ Masters Degree in Occupational Therapy curriculum accredited jointly by the council of medical education of the American Medical Association and the AOTA
+ Licensed and registered by NYS Education Department in Occupational Therapy
+ AOTA Specialty Certification in Low Vision preferred
+ Minimum 5 years of experience
+ NYS driver’s license and reliable transportation OR the ability to travel effectively within assigned geographic area, including the ability to transport equipment
