Clinical Pharmacist Lead

Position Highlights:
* Work location: 1301 Pennsylvania Ave, Fort Worth, TX 76104
* Work hours: Monday - Friday 7:30 am - 4:30 pm day shift only
Pharmacy position highlights:
+ Will function as allied health provider of medical staff with clinical privileges
+ Provide in-person comprehensive medication management under collaborative practice agreements for diverse populations
+ Provide telehealth medication management encounters for specialty pharmacy patients
+ Provide telehealth medication management encounters for chronic disease state patients
+ Precept pharmacy students and pharmacy residents
Here's What You Need
* Bachelor's Degree Pharmacy Required
* Other Pharm D. from US -accredited College of Pharmacy required
* CLIAFE Must comply with CLIA regulations regarding foreign education equivalency required.
* PHAR- Pharmacist upon hire required.
* 2 years of hospital pharmacy experience with expertise or training in specialty are required
* Completion of PGY1 training is required; PGY2 training in ambulatory care is strongly preferred.
* Board certification (BCPS or BCACP) within 30 days of hire is preferred.
* Medical staff credentialing and privileging must be completed within 90 days of hire (preferred).
* Carrying malpractice insurance is preferred.
* Must be proficient in written and verbal English language communication skills, computer proficient and possess basic math skills.
What You Will Do
Team Leadership
* Supports the oversight of employee performance directly and indirectly.
* Advancing employee partnership through servant leadership, including actively recognizing excellence and improvement from co-workers to mentor growth and development.
* Makes recommendations for human resources decisions (hiring, scheduling, merit, discipline).
* Supports and/or assists in the training of newly hired pharmacists.
Department Support
* Assists in coordinating the department's operations
* Serves as a resource for policy and procedure development within designated areas of assigned responsibility.
* Leads departmental projects aimed at improving or sustaining operational performance.
* Represents the department in hospital/system committees/teams/workgroups.
* Actively participates in efforts to maintain regulatory compliance.
* Clinical Pharmacist Role modeling
* Communicating effectively, building positive professional relationships and role modeling teamwork.
* Timely and accurate support of medication distribution including medication order verification and oversight of pharmacy technician responsibilities.
* Provision of clinical pharmacy services for the safe and efficient patient care.
* Ensuring the accuracy of one's own work as well as maintaining required competencies and other documentation. Consistent support of the department's quality improvement and patient safety standards programs and initiatives is also required.
* Ongoing professional development and growth.
* Not only the management of one's own time but shares the responsibility for minimizing pharmaceutical costs (including wastage) and discovering new process efficiencies.
* Respond to medical emergencies per hospital policy as available.
* Other duties as assigned.
