    Job Details:

    HIM Representative II performs a variety of tasks both routine and complex within the Health Information Department including, but not limited to release of information, internal and external communication, and support of various tasks and workflows.

    Responsibilities:

    + Organizes, maintains, and evaluates medical records (electronic or paper).

    + Responds to requests to review medical records/and or retrieve missing documentation.

    + Researches any significant name or medical record changes and notifies management.

    + Ability to analyze complex information and use problem solving skills to determine appropriate solutions and necessary merges to protect the integrity of the EMPI.

    + Corresponds with Network hospitals regarding duplicate number identification and merging.

    + Provides support to patient accounts and/or home care services as needed.

    + Serves as alternative coverage for HIM Representative I and/or various tasks within the department.

    + Assists in training and/or orientation as needed.

    + Provides support to coding and CDI workflows.

    + May assist with reporting functions for regulatory, state, and accreditation agencies.

    + When necessary, merges duplicate medical record numbers in the MPI in a timely fashion.

    + Accesses various systems and maintains departmental dashboards.

    + Pull all medical records requested by hospital departments.

    + Accurately validates computer generated discharge/admission list against the actual medical record and preps inpatient/outpatient medical records for scanning.

    + Accurately performs Quality Control on scanned inpatient medical records to ascertain all paper documents are scanned, and belong to respective patients.

    + Performs analysis on physical documentation requiring dictations, signatures, and co-signatures. Deficiencies are entered into EMR system and reports generated for follow up with physicians.

    + Diligently reviews pending transcribed report in transcription system, makes necessary corrections and exports into EMR system as permanent part of patient chart.

    + Completing purchase requisitions, and following up with the Purchasing Department on deliveries.

    + Provides DNFB reports for the hospital.

    + Provides back up support to HIM staff in the absent of HIM Supervisor.

    + Monitor equipment for proper functioning, and reports issues in a timely manner to minimize work disruption.

    + Answer all phone calls in a timely manner.

    + Assists customers as needed with courtesy and respect.

    + Ensure the privacy and security of patient information.

    + Other duties as assigned.

    QUALIFICATIONS/REQUIREMENTS:

    Experience:

    + 1 to 2 years HIM experience required

    Education:

    + High School diploma or equivalent required, associate or bachelors preferred

    Other:

    + PC Skills – demonstrates proficiency in keyboard operations, Microsoft Office applications and others as required

    + Customer Orientation – establishes and maintains long term customer relationships, building trust and respect by consistently meeting and exceeding expectations

    + Organization – establishing courses of action to ensure that work is completed efficiently; proactively prioritizes assignments and keen ability to multi-task
