RN Care Coordinator
Job Description
We are seeking a Registered Nurse to join an OB clinic and support a Care Coordination team for 32 hours per week. In this role, you will provide patient education, triage, and case management for patients. Additionally, you will manage patients' medical records and refer them to social services and programs as necessary. The clinic serves a diverse patient population, and you will work closely with interpretive services to ensure effective communication.
Responsibilities
+ Conduct intake assessments for the OB clinic and Chronic Disease clinic.
+ Support the Care Coordination team in delivering comprehensive care.
+ Assist the Telephonic Triage team located on-site.
+ Manage medical records and coordinate scheduling.
+ Operate appropriate patient care equipment and manage computerized information systems.
Essential Skills
+ Current Minnesota RN licensure.
+ Current BLS/CPR certification.
+ Experience in intake assessments is strongly preferred.
+ Proficiency in medical records management, scheduling coordination, and case management.
+ Ability to operate patient care equipment and enter computerized information efficiently.
Additional Skills & Qualifications
+ Spanish-speaking skills are highly preferred.
Work Environment
This position is on-site, Monday through Friday from 8:30 AM to 5 PM, for a total of 32 hours per week, with some flexibility in scheduling. The clinic is a community health facility in North Minneapolis, offering services such as medical, dental, social services, lab radiology, and a food shelf. The clinic provides care to a diverse community, including unhoused and uninsured individuals, and you will have the opportunity to support one of the oldest community clinics in the state of Minnesota.

This is a Contract position based out of Minneapolis, MN.
Pay and Benefits
The pay range for this position is $36.00 - $45.00/hr.
