Are you a Credentialed Alcoholism and Substance Abuse Counselor (CASAC)?

CASACs with at least two years of human services experience are strongly encouraged to apply.

Key Responsibilities:

+ Work collaboratively with participants to assess health goals and implement individualized Plans of Care.

+ Complete HARP (Health and Recovery Plan) assessments to connect members with specialized community-based services.

+ Support members in overcoming barriers to healthcare by providing health literacy education and addressing social determinants of health.

+ Assist high-risk members in managing chronic behavioral health, medical, and substance use conditions.

+ Link participants to appropriate health, behavioral health, and community resources.

+ Coordinate with healthcare providers and community agencies to promote primary and preventive care.

+ Assist participants in reducing or eliminating avoidable hospital and emergency room usage.

+ Maintain and manage all aspects of participants Electronic Health Records (EHR).

+ Promote a culture of inclusion, belonging, and respect.

Qualifications:

+ Bachelor's degree in any field with at least three years of human services experience, OR

+ Master’s degree in a human services field with at least one year of experience, OR

+ CASAC certification with two years of human services experience.

+ Proficiency in Microsoft Office.

+ Prior experience with the Medicaid delivery system preferred but not required.

+ Reliable transportation and a valid New York State driver’s license required.

+ Ability to work effectively in a fast-paced environment.
