Work location: Texas Health Arlington 800 W. Randol Mill Road TX 76012
Work hours: Full Time Day shift (40hours, benefit eligible) Monday - Friday, hours fluctuate based on needs and scheduling.
Education
+ Associate's Degree Nursing Required And
+ Bachelor's Degree Nursing Preferred
Experience
+ 2 Years experience in Cardiac Rehab or Cardiovascular nursing required
+ 1 Year Leadership Experience preferred
Licenses and Certifications
+ RN - Registered Nurse Upon Hire Required And
+ BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required
+ ACLS - Advanced Cardiac Life Support 90 Days Required
+ CRRN - Certified Rehabilitation Registered Nurse Upon Hire Preferred
What You Will Do
* Performs age appropriate patient assessment and establishes plan of care
* Obtain/verify physician order for therapy and validate for appropriateness
* Review available medical records and obtain pertinent medical history from patient or family
* Ensure that patient has given informed consent to treatment
* Orients patient and family/significant other to rehab program as required by regulatory requirements
* Coordinates interdisciplinary team process and care of patient that facilitates comprehensive age appropriate assessments of patient?s needs, abilities, and functional limitations
* Ensures patient / family participation in goal setting and discharge planning
* Establishes age appropriate patient care plan that coordinates multiple rehab disciplines into a unified overarching plan of care that adheres to regulatory requirements
* Integrate assessment results to identify functional problems and prioritize care needs with patient and family
* Initiate therapy discharge planning in collaboration with care team and patient based on prognosis and expected discharge disposition
* Demonstrates competence in the performance of age appropriate patient assessment and individualizes care based on age specific needs
* Performs age-appropriate patient treatment.
* Maintains funding authorization for continuation of care
* Coordinates scheduling of Cardiac rehab therapy appointments
* Uses therapeutic treatment techniques appropriate to patient age and desire for treatment
* Performs treatment procedures that are within the scope of the professional license
* Performs/delegates technical procedures appropriately and safely
* Utilizes a variety of treatment techniques to achieve goals, if necessary
* Determine patient response to treatment and evaluate progress (reassessment)
* Educates patient and family through instruction, demonstration, or appropriate handouts in treatment plan, safety issues, home programs/self-management and assistive equipment
* Patient and staff safety maintained throughout treatment
* Communicates with physicians and other healthcare personnel as appropriate regarding patient care issues
* Demonstrates competence in the performance of age-appropriate patient treatment and individualizes care based on age specific needs.
* Demonstrates Professional Accountability
* Utilizes available resources (equipment, support personnel, educational resources) to maximize efficiency and productivity and meet developmental needs
* Participates in orientation and training of student, volunteers, and observers.
* Attends and/or leads hospital, department, and community educational in-services
* Meets established productivity targets for department/program
* Represents department in a positive manner to hospital/system/community
* Completes assigned work within scheduled hours and offers assistance to others to ensure departmental work is accomplished
* Seeks assistance/supervision/consultation as needed and reports to supervisor any problems and/or solutions regarding delivery of patient care
* Willing to be cross-trained to treat any diagnosis or level of care
* Participates in continuous improvement activities and/or teams
* Demonstrates commitment to the maintenance of a clean, safe work environment that is compliant with departmental, hospital and regulatory agency requirements
* Works in partnership with peers, other disciplines, and departmental personnel
* Complies with departmental personnel policies 20%
Performs Appropriate Documentation
* Documents all aspects of assessment and plan of care in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines.
* Documents all aspects of each intake process and treatment sessions in accordance with departmental, The Joint Commission, Medicare, and other applicable regulatory guidelines.
* Accurately submits patient charges and records procedures according to departmental policy.
* Documents assessments of education needs, education provided, and patient understanding of education provided in accordance with departmental, The Joint Commission, Medicare, and other applicable regulatory guidelines.
* Completes discharge summary for each patient in accordance with departmental, The Joint Commission, Medicare and other applicable regulatory guidelines.
* All documentation is completed within established time frame.
* Accurate timely documentation and reporting of unusual occurrences, on the job injuries, and medical device malfunctioning.
* Completes required documentation tools for outpatients and for specialty units/programs.
Leadership
* Coordinates day-to-day operations of the department
* Supervises professional and support personnel for area of responsibility
* Assists with annual performance review of professional and support personnel for area of responsibility
* Meets with staff to coordinate and schedule patient care or discuss problems concerning patient care for area of responsibility
* Monitors daily patient volume versus staffing ratio and spread of patients among available staff to ensure self and staff are meeting productivity targets
* Assists and teaches less experienced staff
* Assists department leadership in the ongoing assessment of customer needs, the assessment and modification of current programs and the development of new programs
* Participates in strategies to meet business initiatives and unit/team, program, and department specific goals
* Markets new programs to appropriate stakeholders both internally and externally for all outpatient therapy services and leads procurement of appropriate collateral
* First to receive and address the management and resolution of outpatient customer complaints
* Assists department leadership in the achievement of KPIs
* Assures data gathering of outcomes for area of responsibility
* Analysis and action planning of outcomes achieved for area of responsibility
* Implements action plans for improvement of performance in areas of responsibility
* Assists with development and procurement of new or updated patient educational materials for area of responsibility
* Acts as a resource for therapists, assistants and support personnel in solving departmental issues
* Demonstrates awareness of entire departmental patient care needs and works with peers to ensure adequate patient care coverage
* Serves as a preceptor in orientation / assessment of clinical competence for new hires
* Promotes departmental image, may make sales calls, and communicates pertinent information to all stakeholders
* Assists in development and ongoing monitoring of capital and operating budgets
* Assists in selection and supervision of Cardiac Rehab employees
