We’re looking for a motivated, detail-oriented, and compassionate individual to join our Centralized Admissions Team, supporting hospital partnerships and helping patients access the care they need.
Position Summary:
Centralized Admissions is responsible for managing the referral review and admission processes for assigned facilities from a central admissions office which will be in Utica & Personal Health Care. This role involves effective communication with facilities and referral partners, ensuring a smooth intake process, and maintaining compliance with regulations while enhancing occupancy levels
Essentia Duties & Responsibilities:
+ Work with Admissions management team to improve and optimize the admission process at Personal Health Care
+ Collaborate with facility administrators and nursing directors to support admission needs at each location
+ Coordinating the resident referral and approval process in an efficient manner
+ Establishing & maintaining relationships with local hospitals, physicians & community organizations for the purpose of generating consistent admissions to the facility
+ Understand the admissions process including policy, procedures, assessments & paperwork requirements
+ Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes
+ Coordinate and implement effective communication with facilities and referral partners regarding admissions and bed availability.
+ Manage the referral intake process, including electronic referrals, e-faxes, and direct inquiries.
+ Review referrals for acceptance based on clinical criteria and communicate admission decisions to referral partners.
+ Confirm payers and partners with the business office on verification of benefits and submission of pre-authorization where required.
+ Maintain knowledge of Federal and State regulations and reimbursement processes (Managed Care, Medicare, and Medicaid).
+ Effectively manage budgeted occupancy levels and enhance census.
+ Alert appropriate department heads and building staff of projected admissions, including orders, special equipment needs, transport time, and room assignments.
+ Maintain a close working relationship with the facility Administrator, Director of Nursing, and Liaison for efficient admission decisions.
+ Maintain a database of medical contacts/community resources and communicate industry trends and regulatory issues to facility staff and community contacts.
+ Complete required data entry of referrals
QUALIFICATIONS:
+ Associate degree in Marketing, Healthcare Administration, or Nursing; bachelor's degree preferred.
+ 2 years of experience as an Admission Coordinator in a healthcare facility or a similar role.
