The Department of Pediatrics, Division of Adolescent & Young Adult Medicine is seeking a full time **_Community Resource Specialist_** (Substance Use & Behavioral Health). Under the direct supervision of the Co-Principal Investigators and the Sr. Research Program Coordinator, the Community Resource Specialist will assist in the day-to-day logistical and operational tasks involving the provision of connections and linkages to community services for adolescents aged 12-17.
Specific Duties & Responsibilities
+ Work with patients with substance use and other behavioral health and complex social issues to identify appropriate resources.
+ Patient population will include adolescents and young adults ages 12-17 dealing with alcohol and drug use and alcohol and drug use disorders and may include other health and behavioral/mental health concerns.
+ Provide resources, support, outreach and coordinated care to patients.
+ Assist interdisciplinary team with crisis intervention, problem solving and idea sharing.
+ Facilitate enrollment and appointments for specialty substance use services and behavioral health care.
+ Manage tracking of utilization resources.
+ Work with other staff to ensure diagnostic and consultative reports are provided to primary care providers and other appropriate staff in a timely manner.
+ Registers and schedules patient appointments and referrals when needed.
+ Access and interpret patient coverage data in EPIC.
+ Screen patients for eligibility and assist those who are applying for assistance programs.
+ Maintain strong working relationships with members of Johns Hopkins Medicine and other organizations for needed assistance.
+ Maintain expertise on various benefit resources that may aid patients in gaining access to substance use, behavioral health and other service delivery systems.
+ Schedules appointment to meet with patients and follow up as necessary.
+ Interpret and obtain understanding of patient coverage information.
+ Receive patient referrals from the JHH health professional staff and interface regularly with medical providers and support staff.
+ Participate in team meetings for the purpose of improving services to patients and maximizing patient benefits.
+ Work with agencies, providers and clinical staff to trouble shoot problems in communication regarding substance use and other specialty care programs.
+ Assist patients with transportation needs/arrangements per guidelines.
+ Maintain patient confidentiality.
+ Assist with clinical administration duties such as patient check out and scheduling follow-up appointments.
+ Other duties as assigned.

Minimum Qualifications
+ High school diploma or graduation equivalent.
+ Two years of related experience.
+ Additional education may substitute for required experience and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.
Preferred Qualifications
+ Fluent in Spanish and English highly preferred.
+ Master’s degree in related field preferred.
+ Training in Social Work, Behavioral Health or Addiction Services highly preferred.
+ Advanced knowledge of adolescent/young adult and family resources and community services.
+ Knowledge of local SUD prevention and treatment, behavioral/mental health, education and other key resources for the target population.

Starting Salary Range: $15.70 - $26.25 HRLY ($47,000 targeted; Commensurate w/exp.)
