    Responsibilities & Duties:

    + Deliver comprehensive primary care services to patients of all ages, to promote preventive health and wellness and treat conditions in a patient-centered setting.

    + Collaborate with a multidisciplinary team to develop individualized care plans, coordinate referrals, and engage in community health initiatives to improve overall patient outcomes.

     

    What We're Looking For

    Candidate Qualifications:

    Education:

    + Graduate of an accredited school of medicine required

    + Completion of an accredited residency program required

    Licensure/Certification/Registration:

    + Current MD or DO license in appropriate state

    + Board certification or eligibility in Family Medicine

    + Current DEA license required
