    THE POSITION

    Join our dynamic healthcare team as a Utilization Management RN, where you will play a vital role in ensuring the effective and efficient use of healthcare services. Collaborate 1:1 with the Transitional Care Clinic (TCC) Physician to promote high-quality clinical care and effective care coordination. Assess and evaluate patients’ medical and psychosocial needs to develop individualized transition plans that ensure safe movement from hospital to home or alternate settings. Coordinate care among interdisciplinary teams—including physicians, case management, virtual care nurses, mobile paramedics, social workers, and therapists—to ensure continuity and prevent readmissions Provide education to patients and families regarding health management, medications, and post-discharge instructions to promote self-management and adherence. Must have licenses to practice as a Registered Nurse in North Carolina and have practiced 5 years in a Medical Surgical setting. BLS/CPR through the American Heart Association required.
