Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado’s veterans. At Fitzsimons, we provide long-term care and short-term rehab and have a secure memory care neighborhood. Description of Job
While in this position, you are required to work at your assigned work location during your scheduled work times. Please note: Work arrangements are subject to change at any time.
This position will perform initial assessments including but not limited to payer source identification/verification, veteran status verification, service connection verification to ensure the individual meets the criteria required by the facility for custodial or skilled nursing care. This individual will work closely with potential residents and their families to ensure they are informed of resources required for the admission process. This position, in conjunction with the admissions coordinator, coordinates the application and admission process and will work with hospital discharge planners, hospital and community social workers, home care agencies, veteran service officers, veteran organizations and other agencies and individuals throughout the State to generate admissions to the facility and coordinate the admission of qualified residents. This position will work with community organizations to solicit their involvement and support of the facility, educate hospital/assisted living/independent living/other community providers about the services offered at Fitzsimons and assist in assessing potential admissions.
Additionally, as the Admissions Coordinator you will:
Act as the initial first impression of Fitzsimons and as an ambassador for Fitzsimons in the community. You will acts as primary point of contact regarding potential admissions and re-admissions providing information on the nursing home to the public regarding eligibility, charges and services to include but not limited to the following:
+ Scheduling appointments, conducting tours, makes follow-up contact as needed to families and referrals sources, and admission generating telephone contacts.
+ Interviews residents and families in person or by phone (at hospitals, homes, or other facilities) and provides a detailed analysis and assessment of the residents payer source, veteran status and other information as needed to the multidisciplinary team to determine appropriateness of placement.
+ Foster relationships with discharge planners and develop new relationships with potential resources for referrals.
+ Ensures that all necessary documentation is completed prior to admission.
+ Prepares welcome bags for new residents, completes new resident welcome protocol, and prepares packets of information as needed.
+ Assists with direct mail campaigns-mail merge, label printing, and envelope stuffing. Assists with preparing articles, photographs and forms for quarterly newsletter.
+ Communicate effectively and build relationships with a variety of community referral sources, including but not limited to, hospital discharge planners/case managers, VA regional office staff, physicians and other stakeholders and organizations as needed throughout Colorado to maximize the number of admissions from these resources. Ensures that all VA and in-house tracking is completed in a timely manner
+ Consults with community resources for admission/discharge planning.
+ Assists with development and maintenance of a detailed referral log, referral source targeting lists, resident wait list and other targeting activities as needed.
+ Participates in a variety marketing activities/initiatives as requested by Marketing/Admissions Director.
+ This position will be responsible for knowing CMS, VA and CDPHE regulations in order to determine if the potential admission is an appropriate placement for the facility.

Experience Only:
Six (6) years of relevant experience in an occupation related to the work assigned to this position
OR
Education and Experience:
A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years.

+ This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position

CDHS Selection Process Explanation
Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
