    Description:

    Under the supervision of a Registered Nurse or Licensed Practical Nurse, works collaboratively with the health care team to provide outcome/goal-based patient care. Observes and reports the patient’s response and progress to interventions. Adheres to MercyOne's performance expectations and performs all defined services and other related duties in accordance with the Mission, Vision and Values of MercyOne.

    ESSENTIAL FUNCTIONS

    + Supports the team in identifying patient needs and implementing the outcome/goal-based plan of care.

    + Assists in monitoring patient condition – (i.e., temp, vitals, blood glucose monitoring, etc.)

    + Assists in educating patients and significant others (i.e., hygiene, positioning, ambulating, etc.) under the direction of the RN.

    + Assists the patient care team in the delivery of care.

    + Sets up rooms to receive patients.

    + Assists in admitting patients and orienting them to the hospital routine.

    + Performs all appropriate functions of the PCA role when needed to function as a “sitter” for a patient.

    + Makes patient call lights on unit a priority and responds promptly.

    + Provides bathing, oral hygiene, toileting needs and assists in feeding.

    + Assists with prevention of skin breakdown by taking the initiative to reposition patients regularly and ambulate as directed.

    + Transports patients safely.

    + Demonstrates competency in unit specific tasks and seeks assistance when necessary.

    + Responds to telemetry calls promptly to assure ongoing monitoring capability.

    + Participates in "hourly round" process to assure patient safety and that patient needs are being met.

    + Adjusts to changes in team/patient priorities as determined by the team leader.

    + Assists others, outside immediate team, when own work assignment completed.

    + Actively participates in team communication to modify the outcome/goal-based plan of care and to keep the Cardex information current.

    + Reports observations of patient(s) responses to care, (i.e., tolerance of ambulation, etc.)

    + Reports and documents accurate patient information such as vital signs, weights, I&O, blood glucose, specimens, calorie counts, etc.

    + Immediately communicates important information to the team leader/RN such as changes in condition, abnormal vital signs, questions about care, family questions or concerns, etc.

    + Documents completely, accurately and legibly and in accordance to policy.

    + Performs unit-based functions as assigned.

    + Completes shift duties as assigned (i.e., cleaning lists, temperature checks, refrigerator temperatures, etc.).

    + Cleans equipment in dirty utility room before end of shift and returns to storage area.

    + Makes rounds in all assigned rooms to assure dirty linen removed, items stocked as needed and room tidy.

    + Removes equipment/supplies from rooms upon discharge.

    + Using his/her knowledge of the principles of growth and development and related needs, communicates and intervenes in a manner appropriate to the age of the patient served which includes:

    + Birth – 28 days Neonate

    + 1 – 18 Months Infant

    + 18 Months – 3 Years Toddler

    + 3 – 6 Years Preschool-aged

    + 6 – 12 Years School-aged

    + 12 – 18 Years Adolescents

    + 18 – 65 Years Adults

    + 65+ Years Geriatric

    + Collects and applies age-specific data.

    + Performs tasks/procedures, uses equipment in an age appropriate manner.

    + Communicates and works with others in an age appropriate manner.

    + Adheres to all safety, infection control and colleague health policies and procedures.

    + Attends required safety programs.

    + Appropriately and consistently uses the personal protective equipment required of the job.

    + • Knows and can demonstrate departmental emergency procedures for fire, disaster and other emergency situations.

    + • Actively participates in regularly scheduled drills (Code Red, Green, etc.) and regulatory safety standards.

    + Completes and appropriately routes incident reports in a timely manner.

    + Uses equipment safely and appropriately.

    + Reports and removes unsafe equipment/furniture.

    + Asks for assistance when needed to assure safety for all involved.

    + Maintains a safe working environment and practices safe working habits.

    + Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

    MINIMUM QUALIFICATIONS

    Education/Training (one of the following):

    + Verification (by transcript or certification) of successful completion of **Nurse Aide I course**

    + Successful completion of **Nursing Fundamentals course**

    + Successful completion of **Trinity Nursing Assistant Program (TNAP)**

    + Successful completion of the **Competency Test**

    + **Iowa Emergency Medical Technician certification**

    + **Preferred:** Nurse Aide II – Advanced course (may be provided post-hire)

    + **Home Health:** Must have completed Nurse Aide I course

    Experience

    + **No prior experience required**

    Certifications

    + **BLS (Basic Life Support)**

    + **Mandatory Reporter**

    + **Non-Violent Crisis Intervention Training (NVCI)** within 6 months of hire (required for Mental Health/CD roles)
