What You'll Do:

As a Social Worker, you'll play a vital role in supporting patients and families through every stage of care. Your responsibilities will include:

+ Conducting comprehensive psychosocial assessments and providing crisis intervention, counseling, and discharge planning.

+ Connecting patients and families to community resources, federal/state programs, and support systems.

+ Utilizing therapeutic methods such as brief goal-directed therapy, crisis intervention, and psychosocial support.

+ Advocating for patients regarding insurance coverage and post-acute care services.

+ Collaborating with healthcare teams to create individualized care plans and educate families on medical decisions, advance directives, and end-of-life care.

+ Maintaining accurate documentation and adhering to professional ethics and confidentiality standards.

+ Education:

+ Master of Social Work (MSW) degree from a CSWE-accredited School of Social Work (required).

+ Licensure & Certifications:

+ Current, active Social Work License OR successful completion of the social work licensing exam within 9 months of employment.

+ Acceptable licenses include:

+ Clinical Social Worker (CSW)

+ Licensed Social Worker (LSW)
