• Home Health Physical Therapist Assistant (PTA)

What You’ll Do

As a valued member of our therapy team, you will:

+ Deliver high-quality physical therapy services under the direction and supervision of a Physical Therapist

+ Support the patient’s Plan of Care and report changes in condition to the supervising PT

+ Ensure care aligns with state practice guidelines and physician orders

+ Help patients achieve measurable progress toward their mobility and independence goals

+ Teach patients and caregivers how to safely use assistive devices

+ Provide education on therapy-related home exercise programs

+ Promote Elara Caring’s mission, values, and commitment to excellence

+ Work in a physical, hands-on environment that requires lifting and mobility

+ Work occasional irregular or on-call hours as needed

What We’re Looking For

+ Graduate of an accredited Physical Therapist Assistant program

+ Current, unrestricted PTA license in the state of practice

+ At least **6 months of PTA experience** preferred (new grads encouraged to apply!)

+ Reliable transportation, valid driver’s license, and auto insurance

+ Willingness to travel within the service area (up to 50%)

+ Ability to lift 50–100 lbs and perform physical tasks related to patient care
