    Certified Occupational Therapy Assistant (COTA) – $40/hr – Multiple Locations – Full-Time & Part-Time

    Position Overview:

    Under the supervision of an Occupational Therapist, the Certified Occupational Therapy Assistant provides occupational therapy services including treatment, implementation, standardized assessment data, and documentation to students in the school setting.

    Assignment Details:

    + Locations: Goodyear, Phoenix, Glendale, Anthem (AZ)

    + Start Date: December 14, 2025

    + End Date: June 2026 (36 weeks)

    + Schedule: Varies by location

    + Hours: Up to 40 hrs/week

    + Duration: 36 weeks

    + Pay Rate: $40/hr

    + Modality: In-person

    + Openings: Multiple

    + Shift Times:

    + Goodyear: Mon–Thu, 8:30 AM – 2:30 PM

    + Phoenix: 2.5 days/week

    + Glendale: Full-Time

    + Anthem: 2 days/week

    Responsibilities:

    + Provide occupational therapy treatment under OT supervision

    + Implement therapy plans and document progress

    + Collect standardized assessment data

    + Collaborate with school staff and therapy team

    + Maintain compliance with professional standards and district requirements

    Requirements:

    + Current certification or licensure as an Occupational Therapy Assistant in Arizona

    + Graduate of an accredited Occupational Therapy Assistant program

    + One (1) year of experience as a COTA preferred

    + Pediatric experience preferred

    + Current CPR (if applicable)

    + TB questionnaire, PPD or chest X-ray (if applicable)

    + Current health certificate (per contract or state regulation)

    + Must meet all federal, state, and local requirements

    + Must be at least 18 years of age
