    We are currently searching for our newest **Population Health Pharmacy Technician** who is passionate about providing outstanding customer service to our community.

     

    **Position Details** :

    + **Location:** Remote

    + **Status:** PRN, hourly

    + **Shift Hours:** 8:00AM - 4:30PM

    + **Weekend Work:** None

    + **On Call:** No

    **Population Health Pharmacy Technician** , under the immediate oversight of the Pharmacist, responsibilities include:

    + Support the mission of the hospital and pharmacy department

    + Notify the pharmacy/provider of missing prescriptions

    + Request medication refills

    + Contact pharmacies to confirm prescriptions were received and picked up

    + Call patients to remind them to pick up their medications

    + Request records from clinics and facilities as needed

    + Will involve data analysis, e.g., payer data, claims, etc., research, and problem-solving to obtain resolution.

    + Send referrals to other disciplines to coordinate care

    + Obtains medication lists and engages in discussions with patients about medication adherence to identify and resolve patient adherence barriers that do not require clinical intervention.

    + Assists the PHSO with medication- related quality measures for the PHSO.

    Required Qualifications:

    + High School Graduate or Equivalent GED

    + Required FL Registered Pharmacy Technician License

    Preferred Qualifications:

    + 1 year Pharmacy Technician experience

    + Pharmacy Technician Certification (CPhT)

    + PTCB certification required within 6 months of hire.
