    Health Advocates Network is currently seeking a **Physician** to work at a facility in **Sacramento** **,** **CA** **.** These are _registry_ positions with our company.

     

    **Pay Rate** : $205 / hour

     

    *1099

    Shift details:

    + 2 Full time

    + 2 PT 20 hours a week

    Key Qualifications:

    + Practicing Evidence-Based Medicine and monitoring physiological and psychological health of all patients while working in a broad range of medical departments including primary care outpatient,outpatient medical housing unit where medium to high risk/high acuity patients are housed.

    + Providing Medical consultation and treating patients via remote and virtual communication methods (web and phone).

    + Interpersonal skills & bedside manner

    + Practice-Based Learning & Improvement + Population Management Goals (Diabetes, Hypertension, Hep C, HIV, End Stage Liver Disease, Warfarin, Colon Cancer, Polypharmacy)

    + Motivational Interviewing and commitment to Patient-centered medical home (PCMH).

    + EMR (EPIC, Gerner, Client Healthcare Centricity Practice Solutions-CPS)+ Microsoft Suite

    + Collaborating with nursing staff to devise and implement care plans.

    + Providing direction to nursing and ancillary teams

    + Medical Ethics

    Licensing + Certifications:

    + Active Unrestricted CA Medical License

    + Active Unrestricted DEA License, with X• Waiver

    + ACLS Certiftcation

    + Board Certifted, Diplomate of American Board of Family Medicine

    + (ABFM)

    + ECFMG Certiftcation

    + National Provider Identifier (NPI)

    Professional Experience:

    + Provided outpatient medical care using evidence-based medicine to highcomplexity patients in a collaborative environment.

    + Exhibited empathetic understanding of patients in a state correctional clinic.

    + Examined, diagnosed, and treated physical and mental disorders.

    + Interpreted laboratory analyses and X-rays

    + Directed ancillary medical personnel

    + Prepared and supervising the preparation of case histories.

    + Analyzed situations accurately and adopt an effective course of action.
