Overview
The VP of Post-Acute Care- Skilled Nursing Facilities is a senior executive responsible for the strategic leadership, operational excellence, financial performance, and clinical quality of three skilled nursing facilities (SNFs). This role drives value-based care transformation, regulatory compliance, census growth, and operational efficiency across the post-acute continuum. The VP collaborates with hospital systems, payers, physicians, and referral sources to position the organization as a preferred post-acute partner.
Job Details
Strategic Leadership & Growth
+ Develop and execute a multi-year strategic plan for the SNF division aligned with
+ organizational goals.
+ Lead market analysis and competitive positioning to increase preferred provider status with ACOs, health systems, and managed care organizations.
+ Negotiate and manage payer contracts (Medicare, Medicaid, managed care, bundled payments).
+ Use operational and clinical data to drive process changes, set measurable targets and monitor results.
+ Build and maintain strong referral relationships with internal and external partners to optimize admissions and transitions of care.
Operational Excellence
+ Oversee daily operations of three SNFs, ensuring consistent performance across all sites.
+ Implement standardized clinical protocols, staffing models, and operational KPIs.
+ Drive labor productivity, reduce agency usage, and optimize supply chain costs.
+ Lead adoption of technology (EHR, telehealth, predictive analytics, remote monitoring) to improve outcomes and efficiency.
+ Lead process redesign and change-management efforts to improve quality, efficiency and experience.
Clinical Quality & Regulatory Compliance
+ Ensure 100% compliance with CMS, state survey agencies, and Joint Commission standards.
+ Achieve and maintain 5-Star CMS ratings across the majority of facilities.
+ Reduce hospital readmissions to below national benchmarks (target: <15%).
+ Oversee infection control, wound care, and rehabilitation programs.
+ Lead mock surveys, QAPI programs, and corrective action plans.
Financial Performance
+ Deliver annual operating margins.
+ Optimize Medicare PDP and Medicaid case-mix index (CMI).
+ Reduce bad debt and denials through revenue cycle improvements.
Team Leadership & Talent Development
+ Build a culture of accountability, clinical excellence, and employee engagement.
+ Implement leadership development and succession planning programs.
Stakeholder & Partnership Management
+ Serve as the primary post-acute liaison with health system C-suite executives and physician groups.
+ Represent the organization in industry associations.
+ Lead bundled payment and value-based care initiatives (e.g., BPCI-A, ACO partnerships).
Requirements:
+ Bachelor’s degree in Healthcare Administration, Business, Nursing, or related field required.
+ Master’s degree (HA, MBA, MSN) from an accredited program preferred.
+ Minimum 10+ years of progressively responsible experience in post-acute care.
+ Multi-site SNF operations experience preferred.
+ Deep expertise in CMS regulations, PDPM, and value-based care models.
+ Licensed Nursing Home Administrator – active or eligible.
+ Expert knowledge of PDPM, QRP, VBP, and state Medicaid programs.
+ Strong financial acumen (P&L management, budgeting, capex planning).
+ Exceptional communication and influence skills with C-suite and frontline teams.
Salary Range
USD $225,000.00 - USD $250,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
