    SUMMARY

    Utilizing clinical experience, coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.

    SCHEDULE: 40 hours

    RESPONSIBILITIES

    + Validates supplemental orders and processes per policy.

    + Remains available to assist in other duties deemed appropriate to the scope of practice of an LPN.

    + Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers’ needs are met.

    + Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee.

    MINIMUM QUALIFICATIONS

    + Graduate of an accredited school of licensed practical nursing

    + A valid NYS license as a Licensed Practical Nurse

    + One year of experience in home care setting or medical office preferred

    REQUIRED LICENSURE/CERTIFICATIONS

    + Graduate of an accredited school of licensed practical nursing

    + A valid NYS license as a Licensed Practical Nurse

    PHYSICAL REQUIREMENTS:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

    The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
