EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 120 impactful programs across Long Island and NYC, is looking for a dedicated full time Mental Health Clinician/Case Manager to join our Mental Health Treatment Court Mental Health Enhancement Project program.
+ Salary: $73,084.80 per year
+ Schedule: Monday- Friday: 9AM-5PM (35hr week)
+ Location: 100 Centre Street Room 1416 New York NY 10013
What drives the core purpose of this job:
The Mental Health Clinician/ Case Manager provides services and identification, screening, treatment planning, case management, and court reporting of eligible mentally ill clients.
What are the key Responsibilities:
+ Act as liaison to the courts and correctional facilities. Provide case-finding and screening services for cases within the Court in order to ascertain appropriate treatment for defendants.
+ Follows and implements a comprehensive screening to effectuate referrals to treatment and accompanies and/or arranges transportation for clients to the appropriate facility when necessary.
+ Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.
+ Assist clients in applying for and receiving public and private benefits.
+ Maintain a caseload and monitor client participation in the treatment program through case management appointments, reviewing progress reports and conferencing cases with treatment personnel.
+ Make periodic visits to treatment facilities and maintain accurate log of visits.
+ Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.
+ Investigate and arrange supportive services for clients.
+ Conducts educational and/or supportive groups.
+ Conduct drug testing on clients of the Program as needed and timely visits to treatment and correctional facilities.
+ Act as a resource to staff members for guidance in treatment planning and treatment needs.
+ Train and supervise interns and volunteer staff.
+ Assists supervisor with administrative tasks related to program management.
+ Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.
Requirements for this Role:
+ Master’s Degree from an accredited college or university in the field of mental health counseling, social work, or a related field with at least two (2) years of relevant experience; or a Bachelor’s Degree from an accredited college or university in the field of mental health counseling, social work, or a related field with at least three (3) years of full time, paid work experience in the field of mental health counseling, social work, or a related field.
+ Good communication skills, both oral and written. Ability to work cooperatively with treatment and criminal justice systems.
+ Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
