UPMC Williamsport is seeking a Infectious Diseases Clinical Pharmacist to lead its Antimicrobial Stewardship Program. UPMC Williamsport is a 224-bed nonprofit, acute care hospital serving north central Pennsylvania. It is part of the UPMC Health System, a 37-hospital network committed to delivering world-class care and advancing clinical excellence.
This role is responsible for daily stewardship activities and local oversight of the program, collaborating with UPMC Health System's Antimicrobial Stewardship leadership and pharmacist network to leverage system resources and implement best practices locally. This is a full-time (40 hour work week) position, shift being 8AM - 4:30PM with no holidays or weekends.
Additional responsibilities include serving as a departmental resource, leading quality and regulatory compliance initiatives, conducting research and practice advancement projects, and providing education and mentorship to pharmacy students and residents.
Key Responsibilities
Antimicrobial Stewardship
+ Perform daily stewardship via prospective audit with intervention and feedback to prescribers.
+ Collaborate with hospital and pharmacy leadership, Infectious Diseases, Infection Prevention, Patient Safety, Clinical Microbiology, and other local leaders to improve antimicrobial stewardship practices and patient outcomes.
+ Participate in UPMC's systemwide Antimicrobial Stewardship pharmacist operations meeting and stewardship collaborative.
+ Serve as the pharmacist delegate to the system Antimicrobial P&T Subcommittee.
Clinical Pharmacy Services
+ Lead assigned patient service lines, clinical areas, and therapeutic programs.
+ Facilitate pharmaceutical care services and medication utilization systems aligned with patient needs and evidence-based practices.
+ Design and implement stewardship activities and restriction/surveillance programs.
+ Deliver direct patient care, including decentralized and service-based programs.
+ Make evidence-based medication recommendations and participate in medical emergency management.
+ Provide discharge medication review, reconciliation, and counseling as appropriate.
+ Complete critical patient monitoring and review profiles to prevent or mitigate drug-related problems.
Education & Mentorship
+ Provide education to providers, nurses, pharmacists, and other healthcare team members.
+ Serve as a preceptor for pharmacy students and residents; develop training experiences and competencies.
+ Participate in the educational experience of pharmacy students during Advanced Practice Pharmacy Experience rotations.
Quality & Compliance
+ Track and evaluate pharmacy programs for operational, quality, and financial efficiency; benchmark against best practices.
+ Identify and implement improvements in the medication-use system to enhance safety, optimize outcomes, and control costs.
+ B.S. degree in Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE accredited College of Pharmacy (PharmD preferred)
+ Current licensure as a Pharmacist in practicing state
+ Completion of ASHP accredited PGY-1 Pharmacy Residency OR 3 years clinical pharmacy experience
+ PGY2 training in Infectious Diseases or equivalent experience preferred
+ Knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments.
+ BPS Board Certification preferred.Licensure, Certifications, and Clearances:
