• Social Worker LMSW- Full Time Temp

    The Social Services Department in Manhattan Division of The New Jewish Home is seeking for a full time temporary Social Worker to join their team! The duration of work is between 4 to 5 months and maybe extended if needed.

    Job Responsibilities:

    + Completing MDS and biopsychosocial assessments.

    + Facilitate care plan meetings & discharge planning.

    + Implement Advanced Directives and End of Life Planning.

    + Completing all required documentation in the medical record ( EMR)

    + Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills

    Qualifications:

    + Demonstrates judgment of safety and confidentiality issues

    + Strong verbal and written communication skills

    + Strong customer service focus required

    + Knowledge of the community resources

    + Social work Long term care experience preferred.

    + Master Degree in Social Work, licensed (LMSW)
