Job Title: Medical Assistant - Population Health - EAST VALLEY
Job Description
The Population Health Medical Assistant supports primary care teams in the network either remotely or embedded within a primary care office in the East Valley to improve compliance on population health initiatives. This includes tasks such as annual wellness visits, quality care gap closure, and managing in-network referrals. The role does not involve direct patient care but includes executing standing orders for patients.
Responsibilities
+ Regularly review patient charts for screening needs and quality gap closures, as well as care coordination needs.
+ Utilize standing orders developed by the primary care physician to place orders for screening and quality gap closure needs.
+ Perform chart preparation prior to office visits for all value-based lives to identify open care gaps.
+ Manage in-network referrals for all value-based lives.
+ Perform member outreach to schedule patient visits for campaigns such as annual wellness visits, well-child visits, breast cancer screening, and colorectal cancer screening.
+ Coordinate follow-up visits for transitions of care for value-based lives.
+ Transfer patient data regarding quality care gaps from clinic medical records to population health management platforms.
+ Proactively work on payer care gap and frequent flier/high-risk reports for all value-based lives.
+ Collaborate with the primary care team and clinical performance consultant to effectively manage value-based lives.
+ Disclose relevant training, experience, and credentials to help patients understand the scope of services qualified to provide.
+ Work collaboratively and cooperatively with team members in physician offices and assigned support teams.
+ Understand ALOS, readmission rates, and gaps in care.
+ Perform other duties as assigned by the supervisor.
Essential Skills
+ Experience with electronic health records and appointment scheduling.
+ Ability to review standing orders for patients.
+ Conduct outreach to patients to promote wellness visits and other screening appointments.
+ Medical assisting and CMA certification.
+ Familiarity with CPT codes, especially CPT-2 codes.
+ Proficiency in Microsoft Office suite, including Outlook, Excel, and calendar management.
+ Experience with quality measures such as care gap closures, pre-visit planning, HEDIS measures, and patient outreach.
+ Strong ability to follow instructions, analyze data, coordinate tasks, and manage workload to meet business goals.
+ Excellent verbal communication skills.
Additional Skills & Qualifications
+ At least three years of experience as a Medical Office Referral Coordinator, Medical Assistant, or Utilization Technician in a Health Plan or Managed Care Organization.
+ Diploma from a Medical Assistant program.
Work Environment
The role will be based out of a central hub but requires floating to different provider offices across Maricopa County in the EAST VALLEY. The schedule includes working from the office in Arcadia on Mondays, from clinics in the East Valley territory from Tuesday to Thursday, and potentially working from a clinic or home on Fridays, depending on clinic hours.

This is a Contract to Hire position based out of Chandler, AZ.
Pay and Benefits
The pay range for this position is $22.00 - $28.00/hr.
